Upper School Independent Study for Physical Education
The Independent Study for Physical Education Form is designed for students who are involved in an organized sport or dance outside of Stone Ridge. It enables those who participate in a sport to qualify for credit following 120 clock hours of practice and competition, not including travel time. The activity must be instructional in nature and practice sessions must be supervised. Students applying for this credit must submit practice schedules and competition information/results prior to receiving credit. Please refer to the Upper School handbook for more information.
ImPACT Letter and ImPACT Consent Form
Please read the ImPACT Letter from Athletic Director Mr. Bryant and print ImPACT Consent Form (Immediate Post-concussion Assessment and Cognitive Testing) to be signed and turned in. More research has been done and increased attention has been focused on the effects of head trauma on the brains of athletes. Some athletes within the Stone Ridge community have experienced concussions and their ill effects. Our goal in bringing this program to campus is to raise awareness by athletes, coaches and parents about the signs and symptoms of concussions. ATC Mrs. Gorbsky will be contacting athletes when it is their season to be tested.
