Current Stone Ridge Families:
Please locate the blue box for the grade your child will be entering in September 2013, and follow the instructions for which physician forms to download, have filled out by your doctor, and deliver to the School's nurse, Alana Pitcher.

New Stone Ridge Families:
See the gold box for the full Physician Health Form Packet to download, bring to your doctor, and deliver to the School's nurse, Alana Pitcher.

Forms are due July 30, 2013. Your cooperation is needed and appreciated.
Parents of students in Grades 7-12 who will play after school sports: Every year we MUST have an updated physical on file or your daughter cannot try out for a sport. Thank you.

New Families

This entire Physician Health Form Packet is required and due by July 30.

Rising Grades K-4

Required:
Medication Authorization Order Form and Maryland Immunization Form - click to download and bring to your physician.

If your daughter has allergies, asthma or diabetes, the Emergency Health Care Plan (managment form) must be filled out by your physician.

Optional:
It is helpful to update the Physician's Examination Form at your child's annual physical, although not required.

Rising Grade 5

The entire Physician Health Form Packet is due by July 30. The full packet is due since your daughter is moving up a Division.

Rising Grade 6

Rising Grades 7-8

This entire Physician Health Form Packet is due by July 30. The full packet is due by this date because most students play a sport in Grade 7 and 8. Your daughter cannot try out for a sport until the Physician Health Form Packet is received by the nurse.

Rising Grade 9

This entire Physician Health Form Packet is due by July 30. The full packet is due by this date since your daughter is moving up a Division. Your daughter cannot try out for a sport until the Physician Health Form Packet is received by the nurse.

Rising Grade 10-12

If your daughter plays a sport:
The entire Physician Health Form Packet is required by July 30. Your daughter can not try out for a sport until the Physician Health Form Packet is received by the nurse.

If your child does not play a sport:
The Medication Authorization Order Form is required.
