Health Forms
The following 3 forms are due by July 31, 2019 if your child is:
- A new student (Preschool - Grade 12)
- A current student moving up to Middle School (grade 5)
- A current student moving up to Upper School (grade 9)
- A student in Grades 6-12 who plans to play a sport. Every year we MUST have an updated Health Assessment form on file or your daughter cannot try out for a sport.
For children age 6 or under, the Blood Lead testing form must be filled out by your doctor. This form needs to only be submitted once.
For all students who have allergies, asthma, or diabetes please fill out the Health Care Plan. This form is to be filled out every year.
For all students who need a medication administered at school, please fill out the Authorization to Administer Prescribed Medications form. This form is to be filled out every year.
Incoming Kindergarten Students
All Kindergarten students will be required to have the following immunizations before beginning school:
- DPT-4 or more doses (Dose 5 required if fourth dose given prior to the fourth birthday)
- Polio-3 or more doses (Dose 4 required if third dose given prior to the fourth birthday)
- MMR-2 doses - many of your children have not had their required 2nd dose
- Hepatitis B-3 doses
- Varicella (Chickenpox) -- 2 doses

This club meets several times a year and is designed to support and educate both the students and their families regarding serious food allergies. The club includes all three Divisions. The Upper School Students are encouraged to assume leadership roles and show by example that students with food allergies can take charge of their lives and make wise and healthy decisions regarding their diet and safety.
Diabetic Support Group
This group is comprised of all students and staff that are dealing with the challenges of living with and managing diabetes. The group meets for lunch several times during the year. The older students serve as mentors and role models to our younger diabetics.

About SAGE
The talented chefs from SAGE Dining Services have transformed lunchtime at Stone Ridge School of the Sacred Heart into a daily culinary adventure. The students are treated to real food, cooked from scratch. SAGE`s seasonal menus are custom designed, reflecting the students` preferences as well as the fresh products available in the market. SAGE`s registered dietitians review and approve each menu to ensure that it offers a wide variety of nutritious choices.

Purpose and Mission
About the Nurse
As a mother of two sons and two daughters, she adheres to an unbiased approach when teaching topics in Human Sexuality and Family Life classes. Mrs. Pitcher is also a Certified Catechist in the Archdiocese of Washington Office of Religious Education. “I find opportunites to teach health and wellness in and out of the classroom every day. The students at Stone Ridge are always ready to learn.”
The Role of the School Nurse
The school nurse works in tandem with the Counseling Departments and Academic Deans to promote a balanced program that furthers the health, wellness and education success of each and every student. The nurse facilitates and supports students in their goals toward improved health and disease prevention. The nurse plans various enrichment programs on pertinent health issues that foster a life long commitment to health and wellness. There is collaboration with teachers and the Administration in identifying physical or emotional concerns that may limit or impede educational growth and achieving the academic objectives of learning.
The school nurse is responsible for collecting immunization data on each student to ensure compliance with the Maryland Department of Health and Mental Hygiene and is responsible for identifying and executing appropriate exclusion in the event of a communicable disease. The nurse is responsible for various childhood screening (Vision and Spinal) in both lower and middle schools.
The school nurse is a member of the Stone Ridge crisis team and participates in the development of emergency response procedures and advocates for a safe school policy. The nurse is also responsible for managing the Stone Ridge Health Committee which together with the Administration develops policies and ensures the health needs of the Stone Ridge community are met.
