Consent for ImPACT Testing

 

Dear Parents,

Throughout your daughter’s first year of participation in athletics at Stone Ridge, we ask for your consent to administer a baseline neurocognitive assessment in an effort to best serve your daughter if she were to sustain a head injury during her athletic career at Stone Ridge. Stone Ridge Assistant Athletic Director for Athletic Training, Mr. Andrew Maguire, will lead the administration of the testing.

Stone Ridge currently uses the ImPACT (Immediate Post-concussion Assessment and Cognitive Testing) software program for neurocognitive testing. Stone Ridge has utilized the ImPACT program since 2009.

In recent years, the awareness, education, recognition, and treatment of sports-related head injuries has improved significantly. Baseline neurocognitive testing has become an essential tool in assisting medical professional with the appropriate return-to-play decisions for an athlete recovering from a head injury as the baseline testing allows for a comparison with a post-injury neurocognitive test to assess the pace of recovery. With the permission of a parent/guardian, the athlete will take a computerized test that assesses memory, reaction time, processing speed, and impulse control through a series of tasks. The test results will serve as a baseline score that will only be used for individual comparison by medical professionals in the event of a future head injury. The ImPACT program remains the most widely used neurocognitive test across colleges and secondary schools nationally. We are excited to continue to offer this service to our student-athletes at Stone Ridge. Our goal is to achieve full participation in the baseline ImPACT testing for all Stone Ridge students participating in athletics in Grades 6-12.

I give my permission for my child to have a baseline ImPACT (Immediate Post - concussion Assessment and Cognitive Testing) administered at Stone Ridge. I understand that my child may need to be tested more than once if suspected of having a concussion. The post-concussion test will be compared to my child’s baseline test, which is on file at Stone Ridge. I understand there is no charge for the testing.

Stone Ridge School may release the ImPACT (Immediate Post-concussion Assessment and Cognitive Testing) results to my child’s primary care physician, neurologist, or other treating physician, as asked for by guardian in writing.

I understand that general information about the test data may be provided to the heads of school, for the purposes of providing temporary academic modifications, if necessary.
